SH isn't a "theme" it is a home screen replacement. On the main page, you will see 3 different versions of SH. The 3rd one says for Android 1.5. (the actual file name says cupcake in it). That is the version you need for your phone.
You can use ANY theme that has built, or build your own. Once installed, if you press Menu > Themes, it will take you to the themes. There are a LOT of them. They have nothing to do with which version of the OS you are using. The difference is that some are built for "low res" like G1, Moment, Eris, etc. Others are built for "high res" like droid, N1, Evo, etc.
Once you have played with it a little bit, you can use pretty much any theme, and just do a few quick changes to make it work on your phone. Actually, Lemon's groupie theme is the only high res theme I have found that I can't modify in under 10 minutes to work on my moment. It works, I just can't get the pictures to look right, because I can't get it sized properly.
As a side note, I believe the screenshot you posted is from a rooted phone. There are certain things you can't do without root. One of them is making the stock Notification bar things. Like making the bar clear and changing the icons on the right side of it. The clock is a widget. Either "Beautiful Home" or "Fancy Widget". The only part that needs a home screen replacement is the bottom bar (with arrow phone +). There are quite a few themes that use that type of thing. Most of the themes that say "sense" in them. Also, the Froyo themes are not much different as far as that bottom bar.
